Job Description:
Role Responsibilities: *Administer medications and perform treatments as ordered by physician. Document appropriately. *Assist with direct resident care as necessary, including personal care, feeding, care of environment changes, and safety. *Ensure recording of Intakes and Output, toileting records, turning and positioning sheets and other documentation in neighborhood. *Complete incident reports and initiate investigations occurring on assigned unit, and forward to RN Nurse Manager. *Notify RN for assessment as needed. *Initiate the transfer of residents to the hospital as necessary and ensure completion of necessary paperwork.

Skills Needed:
Qualified LPN candidates will: *Have graduated a state approved school of nursing *Possess a current license as a Licensed Practical Nurse in New York State *Show the ability to work well with others. *Exhibit a genuine interest in the care of geriatric residents *Have a willingness to perform all duties to support Teresian House “Resident-Centered Care” concept *Possess CPR certification.
