Textbook Loan Form Due: June 1, 2010
** Please note the loan form now contains our web address, and a place for parents to include their e-mail address (optional). Every K student, and students who are new to your school, must be registered with their public school district.
Every
form must be signed by a parent/guardian and certified by the
school.
Please copy the form in its entirety, so parents are aware of the
June 2nd form deadline and to call BOCES to confirm receipt of a
faxed submission (reverse side of form).
Use
one line for each book requested. Do not use any bundle number
ISBN’s.
Please
note that there is a page 2 if necessary.
If
the student’s mailing address is a P.O. Box, also give physical
location of home.
If
faxing, please call (464-5123) to confirm receipt of fax.
Please
note there is a page 2 if necessary.

Ongoing
Please advise us if a student will not be in attendance, so that we do not send books for the student. If a student leaves school at any time during the course of the school year, please advise BOCES; it is the parents’ responsibility to return the books to BOCES or to their school district.
You are a vital link in the textbook loan process, and we thank you for your time and effort!
